Patient e-admission portal coming shortly

If you are being admitted to Macquarie University Hospital, you can complete your patient questionnaire and pre-admission form via a secure and convenient online portal from the end of June 2020.

If you need to visit us again you can simply make any changes that may have occurred since your last admission and resubmit the form.

It is important that you complete the form within 48 hours so our nurses can review your health history and if required arrange for you to visit our pre-admission clinic prior to surgery.

Please ensure you have the following information available when you register.

  • Medicare and or DVA number
  • Health fund membership number
  • WorkCover approval number
  • Pension/concession card
  • GP details
  • Next of kin/emergency contact details
  • Regular medications including vitamins and over the counter medicines.

If you have private health insurance please contact your health fund to ensure you are fully covered for your admission.

If you do not have private health insurance please email estimates@muh.org.au for a cost estimation of your hospital fees. This amount will be paid prior to or on admission to hospital.
