Advanced Care Directives
Planning care for those approaching end of life supports a focus of care from invasive medical intervention towards a palliative approach of symptom management to facilitate dying in a place of choice. Early conversations between doctors and patients about goals of care and appropriate use of life sustaining measures improves the end of life care we can provide.
Reassurance is required to both patients and families that the quality and individualised care associated with the ongoing goals of treatment will continued to be provided regardless of resuscitation being appropriate or not.
The resuscitation plan is an agreed management plan inclusive of decisions about use of life-sustaining treatment and the withholding of treatments.
If you already have an advanced care directive in place, please bring this with you to hospital and give us this information when you arrive. If you would like to discuss advanced care directives on admission please speak to your nurse or doctor.
